CD Requisition Forms

Following the changes to the Controlled Drug Requisition Law (Schedule 2 and 3) which came into effect in 2015, specified healthcare professionals are now required to use a Mandatory CDF10 Requisition Form for the requisitioning of Schedule 2 and 3 controlled drugs, including those purchased from wholesalers.
